If a research study protocol requires UF Health services or UF Health Investigational Drug Services (IDS) and the study subject has never been a patient in the UF Health system, the study team will need to request an UF Health Epic Medical Record Number (MRN).
To request a UF Health MRN, email Karen Poole poolek@shands.ufl.edu, Natalie Brooker nelson@shands.ufl.edu, and Becki Mullins mullib@shands.ufl.edu. This process only takes a few minutes. They will need to have the following information about the participant:
- Full Name
- Date of Birth
- Current Address
- Social Security Number (optional and/or if available)
