New Study Submissions
- ALL new study submissions should use the OCR Temporary Paper Intake Form (Transition).
- Once completed, please email the form to OCR-Intake@ahc.ufl.edu.
- Once your study is under OCR review, please send revised documents for the review directly to your OCR CAB liaison.
Revised Study Submissions
- To change, amend, or revise a previously completed submission, please use OCR Electronic Intake Form.
- You must have a GatorLink ID to access the form.
- If you have a GatorLink ID but are still are unable to access the form, please email OCRfirstname.lastname@example.org.
- Once your revision is under OCR review, please send revised documents for the review directly to your OCR CAB liaison.
