OCR Intake Forms

New Study Submissions

• ALL new study submissions should use the OCR Temporary Paper Intake Form​ (Transition)​.
• Once completed, please email the form to OCR-Intake@ahc.ufl.edu.
• Once your study is under OCR review, please send revised documents for the review directly to your OCR CAB liaison.​

Revised Study Submissions

• To change, amend, or revise a previously completed submission, please use OCR Electron​ic Intake Form​.​
• You must have a GatorLink ID to access the form.
• If you have a GatorLink ID but are still are unable to access the form, please email OCR-intake@ahc.ufl.edu.
• Once your revision is under OCR review, please send revised documents for the review directly to your OCR CAB liaison.​
